The information you need to fill in should have been already discussed with a GM. You should have recieved in the mail a character sheet a GM helped you fill out. Do not change anything on the character sheet. Fill in this registry form with the exact information you were given. Should we find out you have lied or forged or changed something you will be immediately dismissed from "EN". We do keep the original copy of your character sheet to check it by.
